About The Position

This full-time position, working 40 hours a week from 7:00 AM to 3:30 PM, will provide comprehensive internal medicine clinical coverage for heart and vascular floor patients, Monday through Friday, with a required weekend and holiday rotation. Emory Healthcare is an academic medical center serving a high acuity patient population, where pharmacy staff collaborate closely with other healthcare providers in an interdisciplinary team approach to patient care. The Pharmacy at Emory aims to deliver patient- and family-centered medication management, striving to define a new standard of care. The Clinical Pharmacy Specialist I acts as a clinical expert in their specialty area, responsible for direct patient care activities, ensuring appropriate medication therapy, and achieving clinical outcomes for the institution within their specialty. This role involves developing and promoting advanced clinical practice based on evidence, establishing collaborative practice agreements, and ensuring institution-wide appropriate therapy and outcomes.

Requirements

  • Graduate of an accredited school of pharmacy
  • ASHP- Accredited Post Graduate Year 1 and 2 Residency or Fellowship or equivalent experience in the specialty area
  • Active Georgia Pharmacist license
  • Basic Life Support (BLS) Certification in accordance with the American Heart Association required within 90 days
  • Advanced Cardiac Life Support required within 6 months, if directly applicable to practice specialty

Nice To Haves

  • Post Graduate Year 1 and 2 Residency (or Fellowship or equivalent experience) in internal medicine with experience with the heart and vascular patient population is highly desired
  • Relevant Pharmacy Specialty Board Certification is preferred

Responsibilities

  • Provide full internal medicine clinical coverage for heart & vascular floor patients, M-F with a weekend and holiday rotation requirement
  • Practice as a clinical expert in specialty practice area(s)
  • Ensure appropriate medication therapy and clinical outcomes for the institution related to specialty area
  • Develop and promote advanced clinical practice based upon recent evidenced based practice
  • Practice under existing and establish new collaborative practice agreements to enhance pharmaceutical care
  • Ensure institution-wide appropriate therapy and institutional outcomes in specialty area
  • Consistently provide patient-centered, interdisciplinary pharmaceutical care
  • Proactively review and process orders accurately and efficiently
  • Prevent and/or resolve drug-related problems to ensure safety, efficacy, and cost-efficiency by consulting with the ordering prescriber and appropriately documents
  • Perform daily in-depth patient profile review to assess, monitor and recommend appropriate drug therapy as applicable
  • Consistently evaluate, intervene, and document clinical interventions beyond institutional protocols and guidelines
  • Support recommendations with critical analysis of literature
  • Monitor drug therapy regimens and make recommendations or modifications per protocol
  • Lead the development of system nomograms and other guidelines
  • Promote the documentation of clinical activities and adverse drug events
  • Attend medical codes as applicable
  • Develop system policies and processes for proper dosing, storage, handling, dispensing, and/or administration of medications and compounded products
  • Actively participate in development of system clinical practice initiatives
  • Serve as a role model for complex clinical and ethical situations and resolves conflict constructively
  • Develop, use and promote effective patient education materials and techniques for drug therapy, adverse effects, compliance, appropriate use, handling and drug administration education
  • Participate in coordination of transitions of care as appropriate
  • Responsible for service completion and patient satisfaction
  • Engage in external professional activities to maintain clinical competency and knowledge of current trends and practices
  • Develop accredited continuing education activities every 2 years
  • Precept pharmacy students and residents as available
  • Responsible for department training and competency assurance for specialty area
  • Develop and assist in training and education of pharmacy and other disciplines
  • Assist pharmacists seeking to advance their clinical skills
  • Answer drug information questions with in-depth knowledge of research designs, methodologies, and biostatistics
  • Actively participate in scholarly activities in specialty area; Submits publications or national presentations routinely
  • Recognize process failures and actively engage in analysis and improvement efforts
  • Lead medication use evaluation or quality improvement project annually in specialty area
  • Oversee the work performed by pharmacy students, technicians and pharmacists to ensure performance standards are maintained and work is in accordance with state, federal and organizational regulations
  • Document adverse drug reactions and medication variances per policy
  • Review trends and recommends patient safety solutions in specialty
  • Promote new ideas to improve quality or overcome barriers to advance the profession in specialty area
  • Responsible for overall departmental pharmacy financial stewardship for specialty area
  • Lead the development and reinforcement of medication use policies and processes to ensure financial medication stewardship
  • Actively participate in formulary review and medication selection to promote cost savings
  • Support and promote cost-savings initiatives
  • Engage in review of medication insurance authorization status as applicable
  • Maintain and promote understanding of 340b practices as applicable
  • Responsible for oversight of medication use systems in specialty area
  • Lead the development and review of protocols, guidelines, or order sets
  • Participate in multidisciplinary or system Emory Healthcare committee
  • Perform other duties as assigned or requested
