About The Position

Clearway Health is looking for a Clinical Pharmacist that will be responsible for providing comprehensive, patient-centered medication management for pediatric patients receiving care in a multidisciplinary weight management, endocrinology, and obesity medicine clinic. This role focuses on optimizing pharmacotherapy for obesity and obesity related comorbidities, ensuring medication safety, supporting lifestyle interventions, and collaborating closely with pediatricians, endocrinologists, dietitians, behavioral health providers, and nursing staff. The pharmacist serves as a medication expert, educator, and clinical resource for patients, caregivers, and the healthcare team, with a strong emphasis on evidence-based pediatric obesity treatment, medication adherence, and long-term health outcomes.

Requirements

  • A PharmD or bachelor's degree from an accredited College of Pharmacy is required.
  • Completion of PGY-1 Pharmacy Residency or 3+ years working as a clinical pharmacist in direct patient care is required.
  • An active Pharmacist licensure (in good standing) to practice pharmacy in DC or MD with the eligibility to be licensed in both within 90 days of hire
  • Must possess or be able to obtain a State Board of Pharmacy Preceptor’s license by the current state you are applying to within one year of hire.
  • 3+ years of relevant clinical experience in endocrinology, obesity, pediatric ambulatory care or weight management OR 5+ years of relevant clinical experience in pediatric pharmacy
  • Demonstrated expertise in medications, drug interactions, medical procedures, therapies, diagnoses, and disease states is essential.
  • Exceptional interpersonal skills for effective communication with diverse stakeholders including hospital staff, patients, families, physicians, and colleagues.
  • Ability to effectively manage multiple tasks, prioritize duties, and adhere to deadlines.
  • Organized work approach with a proficiency in maintaining accurate documentation.
  • Strong verbal and written communication skills; proficiency in additional languages is advantageous.
  • Self-directed with strong self-management abilities; capable of identifying and addressing issues independently.
  • Proven track record in process improvement and a keen eye for detail and precision.
  • Proficiency in Microsoft Office programs (e.g., Word, Excel, Outlook) and web browsers.

  • Board Certified Pediatric Pharmacist (BCPP), preferred

Responsibilities

  • Participate in quality improvement initiatives related to pediatric obesity treatment outcomes, medication safety, and clinic efficiency
  • Contribute to data collection, outcome tracking, and reporting as required
  • Assist with medication access, including prior authorizations, formulary management, appeals, and patient assistance programs
  • Work closely with specialty and outpatient pharmacies to ensure continuity of care and timely medication initiation
  • Provide guidance on insurance coverage and cost-effective medication options
  • Actively participate in multidisciplinary clinic visits and case conferences alongside physicians, advanced practice providers, dietitians, psychologists, and nurses
  • Provide pharmacotherapy recommendations to the care team based on current pediatric obesity and pharmacotherapy guidelines.
  • Collaborate in the development and refinement of clinical pathways, protocols, and order sets for pediatric weight management
