About The Position

Join University of Chicago Medicine, as a Clinical Pharmacist in the Home Infusion Pharmacy, Registry, located in Calumet City, Illinois. In this role as a Clinical Pharmacist you will assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy intended for infusion in the home or at the infusion site in Calumet City. Through various assignments within the department, they provide support for optimal medication therapy to patients with a broad range of disease states. The Home Infusion Pharmacy is known for providing exceptional assistance when medical needs may be complex and requires special knowledge when collaborating with the medical provider and insurance company. Job Summary Responsible and accountable for the provision of safe and effective drug therapy and other pharmaceutical care services intended to achieve outcomes related to the cure, diagnosis or prevention of disease, elimination or reduction of patient’s symptoms, or arresting or slowing of a disease process, and improve a patient’s quality of life in collaboration with a multi-disciplinary team. The pharmacist also assumes the responsibility for pharmacy technicians that work directly under his/her supervision by ensuring the proper preparation of drug products and managing the workflow.

Requirements

  • Graduate of an ACPE accredited College of Pharmacy
  • Eligible for licensure as a pharmacist in the state of Illinois (complete licensure process within 60 days of employment).
  • Completion of an ASHP accredited PGY-1 Pharmacy Residency required or 3 plus years of hospital staffing experience. Home Infusion experience highly preferred.

Nice To Haves

  • Doctor of Pharmacy or other advanced degree highly preferred
  • Advanced Degree, Masters of Science, MBA, or Doctor of Pharmacy highly preferred.
  • Clinical pharmacy experience in Home Infusion highly preferred.
  • Extensive clinical experience and development may be considered in lieu of formal training

Responsibilities

  • Maintains a standard of performance that is in compliance with JCAHO, ASHP practice standards/guidelines, federal/state licensing acts, and UCH and Pharmaceutical Services Policies and Procedures
  • Interprets, evaluates, and accurately dispenses all drug orders in the patient’s best interest as assessed by appropriateness of drug selection, contraindications, dose, side effects, drug interactions, food interactions, duration of therapy, route of administration, and cost
  • Identifies, resolves and prevents potential and actual drug-related problems. Recommends rational therapeutic alternatives as needed to prevent future problems
  • Accurately checks medication orders that have been entered into the pharmacy computer system by the pharmacy technician: insuring that patient allergies, drug duplication, and drug interactions do not exist which contraindicate therapy and that the drug dose is correct. Enter orders directly into computer system if necessary
  • Checks all medications prior to dispensing for accurate labeling, preparation, and storage. Coordinates all clinical and dispensing workflow that occurs from the home infusion pharmacy
  • Supervises the quality/quantity of work and work performance of the pharmacy technician
  • Participates in the orientation, education, and mentoring of new staff (i.e. pharmacists, nurses, pharmacy technicians, etc.) as needed
  • Performs other duties as requested by senior management
