The Internal Medicine Pharmacist provides patient-centered care to patients. This position must: provide pharmaceutical care to patients to achieve optimal therapeutic outcomes; promote cost-effective drug therapy; provide assistance in educating pharmacy students, PGY1 and PGY2 residents; participate in research projects to improve the outcomes for patients; participate on Medical Center and Department committees; adhere to the general hospital standards to promote cooperative work environment by utilizing communication skills, interpersonal relationships and team building; assist with verification of medication orders and distribution of pharmaceutical products; coordinate and participate in the education of pharmacist and other health care professionals; initiate and/or participate in medication utilization evaluations; assist in developing and implementing treatment protocols in order to improve patient care; assist with medication access, patient education, and discharge follow up phone calls. The pharmacist in this position will rotate between the Medicine Service in the acute care setting and Pharmacy Population Health service in the ambulatory setting. The Population Health responsibilities will largely focus on services aimed to improve chronic conditions and address health equities. This includes supporting a 5-year research project that will study the role of a Pharmacist and Community Health Worker in remote monitoring and management of patients with HTN.
