What you’ll do: Clinical pharmacists provide comprehensive pharmaceutical care to designated service areas or patient populations to achieve optimal patient outcomes and safety. They function as members of multidisciplinary teams to improve medication therapy management. These positions act as drug information resources and their responsibilities include age-specific and/or disease specific consultative medication therapy evaluation, education of both patients and other medical staff, research, precepting pharmacy residents, and participating in quality improvement activities. This position provides coverage for the inpatient and outpatient transplant programs, with select responsibilities as follows: Rounds with team, provides order verification and drug information services for service and unit patients, fulfills CMS and other regulatory and site-specific requirements for patients served, including pre and post-transplant notes, evaluation notes, discharge education, listing and waitlist meetings. Chart reviews patients, both pre- and post- transplant, reconciles medications, provides recommendations for vaccination. Within year one post-transplant, reviews medications and adverse events, updates medication plan, provides follow-up on labs and vital signs as needed via in-person visit. Promote safe and effective medication usage and optimal medication therapy outcomes under a collaborative practice agreement. Oversee appropriate medication utilization and related quality metrics within the covered services. Works closely with transplant coordinators, procurement coordinators, physicians, and nurses within program. Completes evaluations of pre-transplant candidates as outlined by program and/or regulatory requirements. Active involvement in PGY1 and PGY2 pharmacy residency programs.
