About The Position

Clinical Pharmacists are responsible for providing safe, effective, and efficient medication management for specialized patient populations. They support medication-use systems and deliver optimal therapy while working with physicians, nurses, and other care team members. Clinical Pharmacists ensure the integration of pharmacy services into patient care by assessing patient needs, adjusting therapy, and preventing medication-related issues. They also serve as departmental resources, conduct research, and support quality improvement initiatives. Additionally, they provide education, mentor pharmacy students and residents, and contribute to initiatives that enhance clinical pharmacy services and regulatory compliance.

Requirements

  • Graduate of an ACPE accredited College of Pharmacy
  • B.S. degree in Pharmacy Doctor of Pharmacy (PharmD) degree; PharmD preferred.
  • Current licensure as a Pharmacist in practicing state
  • Completion of ASHP accredited PGY-1 Pharmacy Residency or 3 years clinical pharmacy experience
  • Knowledge and expertise in advanced pharmacy practice and service, comprehensive medication management, and evidence-based medicine in specialized area appropriate for clinical assignments

  • BPS Board Certification preferred.
  • Transplant experience preferred.
  • TPN experience preferred.

Responsibilities

  • Deliver direct patient care through real-time medication management in emergent and high-complexity clinical scenarios
  • Facilitate acute interventions including medication procurement, preparation, and therapeutic monitoring
  • Provide timely drug information and evidence-based recommendations
  • Participate in emergency medicine-focused research, quality improvement, and practice advancement initiatives
  • Precept pharmacy students and residents; contribute to ongoing education of clinical staff
  • Leads patient service lines, clinical areas, and therapeutic programs.
  • Ensures alignment of drug utilization with patient care needs, evidence-based practices, and regulatory standards.
  • Designs and implements stewardship and restriction/surveillance programs.
  • Tracks and evaluates pharmacy programs for operational, quality, and financial efficiency, benchmarking against best practices.
  • Identifies issues for clinical research, quality improvement, and healthcare provider education.
  • Develops policies for drug purchasing, usage, distribution, and control.
  • Ensures pharmacy integration into healthcare delivery and supports service expansion.
  • Delivers direct patient care, including decentralized and service-based programs, and identifies high-risk patients.
  • Communicates effectively with healthcare providers and participates in stewardship activities and medication programs.
  • Facilitates experiential education, preceptorship, and develops training opportunities for students, residents, and staff.
