SUMMARY: Brown University Health's ambulatory clinical pharmacy team is seeking a pharmacist specialist with pharmacogenomic experience. As a part of our team, you will be responsible for contributing to this innovative service by identifying and managing patients throughout the system and optimizing their therapies based on pharmacogenomic data. In addition, you will ensure safe and effective use of medications, and collaborate with our healthcare teams to achieve positive patient outcomes. Hybrid on-site/remote work schedule available. Reports to Director Specialty Pharmacy, or Ambulatory Manager as applicable. Under the supervision of the Pharmacist-In-Charge, Senior Clinical Pharmacist Specialist, or Clinical Coordinator as appropriate coordinates pharmacy clinical programs including, but not limited to, pharmacotherapeutics, pharmaceutical care, drug information services and pharmacoeconomics. Collaborates with multidisciplinary care providers to monitor, assess, recommend changes, and optimize medication use in area of therapeutic specialty. Serves as a subject matter expert and real-time go to resource in area of specialty. Participates in the development of therapeutic drug policy and implements corrective strategies necessary to contain drug costs in specified patient settings and/or within therapeutic classes. Monitors pharmacy clinical services through continuous quality improvement activities to ensure that customer expectations are met or exceeded. Provides input for the development and implementation of policies, procedures, and budgets for the pharmacy services clinical practice setting.
