The Clinical Pharmacist / Pharmacy Clinical Specialist – Cardiology practices within the Heart, Vascular & Thoracic Institute (HVTI) at Main Campus, supporting a highly specialized team of pharmacists aligned to advanced cardiovascular service lines. This role provides pharmacotherapeutic management for complex patient populations, including general cardiology, cardiothoracic surgery (pre- and post-operative), electrophysiology, vascular, and heart and lung transplant patients. Caregivers participate in interdisciplinary rounds, medication optimization, and cross-training in general inpatient and IV room operations. The Clinical Pharmacist supports a mix of centralized and decentralized responsibilities with rotational coverage, while the Clinical Pharmacy Specialist practices in a primarily decentralized model with a high level of autonomy in specialty areas. This position offers the opportunity to collaborate with world-class providers, work with highly complex cases, and contribute to innovative, leading-edge cardiovascular care. A caregiver in this position will work 7:00am to 3:30pm with weekend rotations occurring every 8th weekend. Rotating evenings shifts are required as well 1 holiday per year - alternating between major and minor holidays annually.

Requirements

  • PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy
  • Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire
  • For individuals required to administer immunizations: current Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire
  • For individuals required to respond to adult or pediatric cardiopulmonary arrest events: current Advanced Cardiovascular Life Support (ACLS) certification or Pediatric Advanced Life Support (PALS) certification through the American Heart Association (AHA) or American Red Cross or certification obtained within one year of hire
  • Completion of a Health-system Pharmacy Administration and Leadership (HSPAL) PGY2 Residency OR completion of either a post-graduate year 1 (PGY-1) or post-graduate year 2 residency (PGY2) and one year of relevant pharmacy experience OR three years relevant pharmacy experience OR for a Clinical Pharmacy Specialist PGY2 residency or fellowship training required; or PGY1 residency plus minimum of 2 years of relevant specialty experience; or Minimum of 4 years of relevant specialty experience.

Responsibilities

  • Lead and coordinate work activities for assigned practice areas and provide feedback to caregivers regarding performance under the direction of the manager.
  • Train and mentor caregivers and residents and precept students.
  • Participate in interviewing and recommend selection of candidates.
  • Triage issues to management and follow-up.
  • Participate in operational tasks such as workflow oversight, process decision making, project implementation, and drug shortage management.
  • Ensure and maintain quality control in regard to patient care and medication and equipment usage.
  • Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems.
  • Advise prescriber of medication related issues and recommend formulary alternatives when appropriate.
  • Review, interpret, clarify, enter and verify patient's information and medication orders for processing.
  • Assist with health-system level decision-making related to practice area, committee work, and decision making for relevant policies.
  • Provide drug information support for pertinent drug information questions.
  • Provide management support and demonstrate leadership for assigned practice area.
