This position involves interpreting and verifying orders, accurately compounding and dispensing pharmaceuticals, monitoring drug therapy, and providing clinical consultation. The role also includes documenting clinical activities, supporting formulary and rational drug therapy, providing medication information, assisting with clinical services, and following up with medical staff on drug therapy issues. The Clinical Pharmacist will also support quality improvement, risk management, and patient safety programs, document errors, participate in training, maintain professional competency, provide technical support for automated systems, supervise pharmacy operations, comply with schedules and on-call duties, participate in disaster response, adhere to the Code of Conduct, maintain controlled substance records, comply with regulatory standards, prioritize work, manage medication waste, maintain outpatient pump documentation, train and maintain USP 797/800 records, follow REMS program requirements, and comply with ASCO QOPI standards and 340B program requirements.
