About The Position

The Clinical Pharmacist is responsible for oncology infusion support, inpatient operations, and clinical staffing. This role provides clinical pharmacy services in an outpatient and/or inpatient practice area, focusing on drug therapy management and assessment for patients. The pharmacist also supports established programs such as drug formulary, pharmacy education, quality improvement, and research related to drug therapy.

Requirements

  • Doctor of Pharmacy, minimum.
  • ASHP PGY-1 residency, or two years of related experience, minimum.
  • Minimum, Pharmacist license and controlled substance license issued by the Michigan Board of Pharmacy.
  • Basic Cardiac Life Support (BCLS) certification may be required (at discretion of manager).

Nice To Haves

  • Board of Pharmacy Specialty certification preferred.

Responsibilities

  • Provides drug therapy management and assessment.
  • Identifies and resolves potential and actual drug related problems.
  • Assists physicians in designing dosing regimens based on patient-specific dosing parameters.
  • Communicates and collaborates with physicians and other clinicians to develop drug therapy policies, protocols, and guidelines.
  • Provides support to the Pharmacy & Therapeutics Committee, which may include serving as a member and/or activities such as preparing formulary drug reviews, medication usage evaluations, policies and guidelines for drug therapy, etc.
  • Participates in performance improvement initiatives and research as appropriate.
  • Participates in departmental, multidisciplinary, hospital and health-system committees as leader, member or consultant.
  • Participate in the training, education, precepting, and evaluation of pharmacy students and/or residents as assigned.
  • Provides education and information on pharmaceutical care and drug therapy to patients, pharmacy staff, physicians, nurses, students and other clinicians.
  • Performs other related duties as assigned such as quality assurance reports, preparation for regulatory inspections, etc.
