The Clinical Pharmacist II will work at Scripps Mercy Hospital San Diego, serving central and downtown San Diego. As the Clinical Pharmacist II your duties will include: Primarily clinical based practice demonstrates ability to perform competencies and prescriptive authorities as approved by the medical staff. Participating in direct patient care with patient care teams. Performing all clinical pharmacy activities and oversees clinical activities of clinical pharmacists in training. Round with the medical team and provide patient care interventions Precepting students and/or residents as appropriate Reviewing and evaluates individual drug orders for appropriateness, safety, and cost. Documenting the care provided to patients. Supervising the drug distribution operation\: responsible for assuring safe and effective alternatives as appropriate. Routinely communicating with physicians, nurses and patients regarding drug therapy. Contributing to the accomplishment of initiatives to improve the cost-effectiveness of drug therapy. Demonstrating ability to provide age-specific patient care as established for area and job title. Demonstrating ability to perform competencies and perform activities as outlined in Pharmacist Authorities policy.
