About The Position

SUMMARY: Ensure consistent high-quality pharmacy care management in an ambulatory care setting. Consistently applies the Brown Medicine (BMED) values of patient care priority, dignity, collaboration, integrity and quality in support of the BMED mission to deliver compassionate, high-quality patient care, research excellence and outstanding physician education. PRINCIPAL DUTIES AND RESPONSIBILITIES: Brown University Health employees are expected to successfully role model the organization’s values of Compassion, Accountability, Respect, and Excellence as these guide our everyday actions with patients, customers and one another. Recommends pharmacotherapeutic regimens and corresponding monitoring plans to prescribers in way that is systematic, logical, and secures consensus from prescriber and patient; utilizes patient interview/assessment skills, current literature-supported evidence and practices, drug initiative information, and established protocols. There will be a focus on high-risk, complex patients Participates in disease state management protocols and assists in developing care plans in collaboration with BMED physicians and nurse care managers. Monitors data and modifies pharmacotherapeutic plans as necessary Perform limited physical examinations, such as blood pressure and glucose measurement, for the purpose of providing medication management services Documents pharmacy care activities in patients' medical records or clinical information system Interacts with and helps educate fellows, residents and students training in internal medicine Interacts with pharmacists in other settings to help ensure care coordination and medication reconciliation Participates, as appropriate, in the implementation of drug initiatives or other formulary management activities Identifies cost effective strategies for obtaining medications Provides clinical oversight to pharmacy technicians and interns Precepts URI College of Pharmacy IPPE/APPE students Performs e-prescription refills and prior authorization submissions Other duties as assigned Performs other duties as assigned.

Requirements

  • PharmD
  • licensed Pharmacist, State of Rhode Island
  • Three (3) to five (5) years of experience in a variety of pharmacy care settings, including outpatient medicine, community, and hospital.
  • Ability to create systems and improve processes
  • Ability to work collaboratively as part of a health care team
  • Ability to communicate effectively and in a supportive manner with patients and prescribers regarding medications and disease state management
  • Highly organized
  • Exercises sound judgment and decision-making.
  • Able to assess and differentiate priorities.
  • Excellent written and verbal communication skills
  • Able to maintain confidentiality with all aspects of information (including patient data) in accordance with the practice’s philosophy and policy and state and federal regulations.
  • Must handle the most sensitive and confidential matters with the utmost discretion.
  • Proficiency with computer skills (i.e., Microsoft Word, Excel and Outlook, and Web-based applications)

Nice To Haves

  • Post graduate training; residency or fellowship preferred
  • Experience with outcome and quality-based programs preferred
