About The Position

The Clinical Operations Nurse (CON) provides clinical leadership, coaching, and operational support across assigned markets to ensure consistent delivery of safe, high-quality dialysis care. The CON serves as the bridge between frontline nurses, regional management, and corporate quality teams - providing real-time clinical guidance, hands-on training, and ensuring compliance with Concerto’s quality, regulatory, and operational standards. This role is a key driver of clinical excellence, survey readiness, and nursing staff development across all assigned facilities.

Requirements

  • Registered Nurse (RN) - active, unrestricted license in assigned state(s); additional multistate or compact licensure preferred.
  • Current CPR certification required.
  • Minimum of 3 years dialysis nursing experience, including 1–2 years in a leadership, educator, or preceptor role.
  • Demonstrated success in clinical audits, survey readiness, and quality improvement initiatives.
  • Strong understanding of CMS Conditions for Coverage and ESRD Network requirements.
  • Excellent communication, teaching, and coaching skills.
  • Proficiency in Microsoft Office, EMR systems (Renvio, Ascend), and data analysis tools.
  • Ability to independently problem-solve and make sound clinical decisions in fast-paced environments.
  • Work includes both computer-based activities and field engagement with dialysis and skilled nursing facilities.
  • Travel up to 60–80% is expected within assigned markets.
  • Must be able to sit, stand, and travel for extended periods.

Responsibilities

  • Reviews outlier labs and ensures timely interventions are executed, providing feedback, remediation, and follow-through.
  • Provides ongoing education and in-services to nurses and PCTs on Concerto policies, clinical protocols, and regulatory standards.
  • Provides direct support during surveys, including being onsite when possible.
  • Steps in to provide leadership coverage in the absence of a Regional Manager (RM) or when requested by leadership.
  • Performs other related duties and responsibilities as assigned; responsibilities are not solely limited to those listed above.
  • Identifies and addresses market-level clinical outlier trends, develops action plans, and tracks follow-up to closure.
  • Provides minimum biweekly updates on unit performance and monthly written summaries of strengths and opportunities, copying the VPO/RDO as required.
  • Collaborates with physicians on outlier patients and clinical interventions.
  • Participates in corporate and regional initiatives, pilots, and special projects.
  • Works closely with dietitians and social workers to address barriers to care, including nutrition and psychosocial challenges.
  • Acts as a hands-on clinical leader and primary resource for facility RNs.
  • Reviews and interprets lab values (Hgb, TSAT, ferritin, Kt/V) and ensures timely interventions with nursing staff and physicians.
  • Ensures critical lab results are addressed, documented, and escalated appropriately to physicians, facility leaders, and Regional Quality and Regulatory Managers (RQRMs).
  • Monitors long-term catheter use and collaborates on AVF/AVG placement and catheter removal plans.
  • Reviews data accuracy and completeness, ensuring patient records and clinical metrics reflect true performance.
  • Conducts monthly clinical audits, including “red clinic” audits and facility observation reviews.
  • Collaborates with Clinical Operations leadership to ensure all assigned facilities maintain continuous survey readiness.
  • Develops and/or conducts corrective action plans following internal audits, surveys, or adverse findings.
  • Educates staff on survey processes and reinforces Concerto’s regulatory expectations.
  • Participates in QAPI, Clin Ops, and outlier meetings, collaborating closely with compliance/quality and operations teams.
  • Serves as a Renvio and Ascend super user, supporting staff in EMR documentation and troubleshooting.
  • Assists with signing off on initial and annual RN and PCT skills checklists and verifies competency documentation.
  • Coaches RNs and PCTs in anemia management, adequacy troubleshooting, vascular access monitoring, and infection control practices.
  • Conducts monthly check-ins with facility RNs and escalates barriers or opportunities for improvement.
  • Provides hands-on support in addressing clinical and workflow inefficiencies.
  • Promotes a culture of accountability, teamwork, and patient-centered care at every site.
  • Provides backup staffing support as necessary to maintain safe and effective operations during call-outs, vacancies, or survey activity.
