About The Position

Under the direction of the Medical Director or Nurse Manager, the Clinical Nurse Specialist - Group Homes – Group Homes oversees and supervises the medical care and medication administration in Level-3 licensed residential programs to meet State regulations for training, observation and oversight.

Requirements

Graduate of an accredited nursing program with an active nursing license in the State of Vermont
Minimum of 5 years of nursing experience
Proficiency in Microsoft Office (including Outlook)
Experience with electronic health record (EHR) systems
Strong written communication skills
Valid driver’s license

Responsibilities

Provide medication administration oversight.
Review and update Group Home Policies and Procedures regarding medication administration.
In person nursing assessment of new admission.
Review medical appointments and update EMR as needed.
Review and update Nursing Care Plans.
Provide staff training for any special procedures and general care of clients.
Assess each individual client weekly.
Identify concerns and work with teams to find appropriate care/solution.
