About The Position

The Clinical Nurse II is a registered professional nurse who is responsible and accountable for the overall management of patient care on a specific unit and shift. As a participatory patient care giver exemplifying leadership, he/she maintains comprehensive knowledge of patients' condition and technical aptitude for providing safe, effective, and individualized emergency nursing care using the nursing process. Performance of duties requires the following skills: Problem solving abilities, delegation, communication, effective working relationships, priority setting and demonstrated leadership ability.

Requirements

  • Licensed as a Professional Registered Nurse in the State of Illinois, is required (Must provide proof at time of interview if already obtained)
  • Three (3) years of recent Emergency Department nursing experience, is required
  • Current Advanced Cardiovascular Life Support (ACLS) certification, is required (Must provide proof at time of interview if already obtained)

Nice To Haves

  • Bachelors of Science in Nursing from an accredited college or university, is preferred
  • Current Pediatric Advanced Life Support (PALS) certification, is preferred
  • Active Emergency Communication Radio Nurse (ECRN) certification, is preferred
  • Bilingual in English/Spanish, is preferred

Responsibilities

  • Performs admission and on-going patient assessment
  • Adheres to the National Patient Safety Goals.
  • Ensures admission assessments, ongoing monitoring, and scheduled reassessments are done according to divisional unit standards.
  • Ensures the completed assessment includes nursing diagnoses to provide age specific care relative to the following factors: cultural and spiritual inclusion, biophysical, psychosocial, environmental, self-care capabilities, safety and infection control, learning needs, and discharge planning needs.
  • Ensures there is collaboration with patient/family and other healthcare team members when planning nursing care during the emergency room visit.
  • Ensures the nursing plan of care is communicated to patient/family.
  • Ensures completion of patient discharge, admission, and transfer assessment.
  • Facilitation of emergency medicine metrics including but not limited to: Left Without Being Seen (LWBS), Door to EKG, Door to Triage, Door to Admission.
  • Ensures implementation of the nursing plan of care
  • Ensures performance of interventions in a competent, skillful and safe manner.
  • Ensures prioritization of interventions according to patient needs.
  • Ensures application of principles of growth and development across the life span when providing age and developmentally appropriate care relative to the following populations: neonate/infant, child, adolescent, adult, and geriatric.
  • Ensures education is provided to patient and family based on assessed learning needs.
  • Ensures provision of interventions to meet patient's identified discharge needs.
  • Ensures evaluation of the effectiveness of interventions and revises as appropriate
  • Ensures that the staff evaluates and reevaluates patient's responses to interventions.
  • Ensures the staff modifies and implements interventions as appropriate.
  • Ensures appropriate documentation
  • Ensures the staff records pertinent information, (assessment/reassessment/on-going monitoring, plan of care, interventions and patient's responses).
  • Ensures documentation is signed legible with full name and classification.
  • Ensure safe delivery of patient care
  • Ensures performance of interventions in a competent, skillful and safe manner.
  • Ensures staff adheres to the principles of BSIS, isolation and asepsis.
  • Ensures staff performs the technical aspects of care according to established practice standards, (procedures, treatments, IV blood administration, medication administration, and IV cannulation phlebotomy procedure).
  • Ensures staff demonstrates competence in the safe and effective use of clinical equipment.
  • Ensures cardiopulmonary resuscitation and other life-saving interventions are performed appropriately.
  • Ensures patient care area is free of environmental hazards.
  • Demonstrates knowledge of decontamination.
  • Applies knowledge of standard operating policies and procedures.
  • Delivers safe, effective, individualized care utilizing the nursing process
  • Demonstrates ability to perform assessment/reassessment and ongoing monitoring of assigned patients
  • Identifies nursing diagnosis based on the assessment data
  • Develops nursing plan of care based on assessed needs.
  • Applies principles of growth and development across life span in providing care appropriate to patient's age specific needs in the neonate/infant, child, adolescent, adult and geriatric client.
  • Performs interventions in a competent, skillful and safe manner.
  • Prioritizes interventions according to patient's needs.
  • Provides education to patient and family based on assessed learning needs.
  • Performs transfer assessments when the patient is 1ransferred in or out of the department.
  • Provides discharge education when the patient is discharged from the emergency department.
  • Provides interventions to meet patients identified discharge needs.
  • Evaluates effectiveness of care and revises interventions as appropriate
  • Evaluates and re-evaluates patient's responses to interventions.
  • Modifies and implements interventions as appropriate.
  • Documents appropriately
  • Records accurately, concisely, and legibly in black ink or in the electronic medical record the following information, assessments/reassessments/ongoing monitoring, plan of care, interventions and patient s responses.
  • Signs documentation with legible full name and classification.
  • Demonstrates leadership/management ability
  • Demonstrates competence in making patient care/staff assignments.
  • Establishes priorities based on the changing patient care needs.
  • Considers needs of orientees/preceptors in making patient care assignments.
  • Assists in identifying staffing needs to ensure safe, quality patient care.
  • Plans, coordinates and assists staff in patient care /department
  • Identifies and resolves patient care problems in a timely manner
  • Follows the chain of command in referring problems.
  • Ensures patient classification is completed in a timely manner.
  • Assigns non-nursing tasks appropriately.
  • Demonstrates effective interpersonal relationships with all health care team members, patient and families.
  • Promotes teamwork and professional relationships among the members of the nursing profession.
  • Ensures patient care needs are met.
  • Demonstrates knowledge of hospital-wide services and makes appropriate referrals as indicated.
  • Participates in/facilitates, quality assessment, improvement activities, department policy/procedures, department rounds, patient care conferences, handoff communication and staff meetings
  • Assists in ensuring that adequate supplies, medication and equipment are maintained.
  • Assists nurse coordinators appraisal of staff.
  • Promotes patient/family centered environment.
  • Works cooperatively with nurse coordinator to accomplish departmental goals.
  • Collaborates with other healthcare members to foster team spirit.
  • Demonstrates effective communication skills
  • Demonstrates ability to communicate accurately, clearly and concisely, orally and in writing.
  • Demonstrates caring and respectful attitude in verbal and nonverbal communications.
  • Communicates pertinent information to patient/family, physician, nurse, and other healthcare team members as appropriate.
  • Provides concise and accurate patient care information during handoff communication.

  • Participates actively in staff orientation and staff development activities
  • Performs as resource nurse/preceptor/mentor when necessary.
  • Assists new staff with information or directions when requested and offers assistance as needed.
  • Conducts cardiopulmonary resuscitation (CPR) drills, fire drills and patient centered conferences or other department educational activities as requested or assigned.
  • Assists in identifying and planning for educational needs of staff including the orientees.
  • Facilitates attendance of staff to schedule educational activities and meetings.
  • Recognizes own learning needs, and seeks opportunities to meet them.
