Under the general supervision of the unit's director or designee the CNC assumes responsibility for direction and coordination of all functions in the unit on his/her designated shifts. In collaboration with other members of the management team, this individual is responsible for the ongoing assessment of the quality of patient care services provided in the unit. The CNC in Discharge coordinates and delivers high quality, patient-centered care by overseeing the effective management of patients into and out of the acute care hospital setting. The CNC utilizes specialized assessment skills to ensure quality admission to the appropriate care areas, and also safe discharges of acute care patients requiring education and post hospitalization care. The CNC serves as an advocate for patients/families/caregivers and models a commitment to the organization's vision/mission/values to support an unparalleled patient experience and optimum clinical outcomes.
