About The Position

Moorings Park is looking for a Clinical Manager for Home Health. The Clinical Manager is responsible for ensuring quality and safe delivery of home health services and coordinating home health care services that reflect the Moorings Park philosophy and home health standards of care Coordinating with the Director of Quality Management. The Clinical Manager plans, develops, implements, and evaluates health care services, programs, and activities on an ongoing basis. Abides by and follows established policy and procedures as outlined by the community, local, state and federal regulations. Requirements

  • Excellent oral and written communication skills in the English language required
  • During emergencies or natural disasters may be required to be on duty for the duration of the disaster.
  • Must have one of the following qualifications: licensed physician, physical therapist, speech language pathologist, occupational therapist, audiologist, social worker, or a registered nurse.
  • Minimum 2 years of home health agency supervisory experience required.
  • Knowledge of State and Federal regulations required.
  • A current Florida’s driver’s license, clean driving record and proof of insurance are required.

Responsibilities

  • Provides oversight of registered nurses, licensed practical nurses, therapists, home health aides, and office staff
  • Demonstrating knowledge of federal and state regulations
  • Monitoring and documenting performance appraisal of home health staff members in accordance with Moorings Park policy and procedures.
  • Resolving grievances of subordinates according to established policies with support from Human Resources.
  • Reviewing the assignment of new referrals and all patient and personnel assignments.
  • Ensuring productivity is met for full-time clinicians
  • Coordinating referrals and patient care, reviewing new referral clinical information in a timely manner.
  • Assures the development, implementation, and updates of the individualized plan of care.
  • Oversees the assignment of new referrals, assuring timely initiation of care based on physician orders and anticipated patient care needs.
  • Assures coordination of care among staff, including conducting multi-disciplinary patient case conferences, as appropriate.
  • Assures completion of the initial assessment and completion of documentation.
  • Assures all documentation, including OASIS, is completed thoroughly and timely.
  • Completes plan of care with F2F information ensuring accurate documentation.
  • Assesses appropriateness of recertification by staff case conference and ongoing review of clinical records each 60-day period.
  • Participates in the QAPI program and strives to ensure patient satisfaction.
  • Assures the confidentiality of client data is maintained in compliance with HIPAA regulations and standards.
  • Provides departmental orientation for new partners and assists the Director of Quality Management with developing in-service programs.
