Requirements
- Active Registered Nurse (RN) license in Pennsylvania
- Graduate of an accredited nursing program
- Current CPR certification
- Strong organizational and communication skills
- A reliable vehicle and valid driver's license
Nice To Haves
- 3+ years of home health experience (preferred)
- 2+ years in leadership or management (preferred)
Responsibilities
- Oversee all patient care operations and clinical staff
- Coordinate assignments, referrals, and care plans
- Ensure patients' needs are continually assessed and documented
- Support and mentor nurses, therapists, and aides
- Partner with physicians and families to deliver the best possible outcomes
- Champion QAPI initiatives and compliance standards
- Lead with compassion, accountability, and vision
