About The Position

The Home Health & Hospice Agency Liaison acts as the main clinical and operational connection between North Country Home Health & Hospice Agency, NCH affiliates, referral sources, and community partners. This position focuses on enhancing relationships with hospitals, providers, skilled nursing facilities, and community stakeholders to ensure effective care transitions, timely referrals, and accurate understanding of program eligibility and services. The Liaison will engage with discharge planners daily, attend relevant meetings, and offer immediate support to providers, care managers, and discharge teams.

Requirements

  • ASN, Associates of Science in Nursing
  • Registered Nurse (RN) with a valid license
  • Association accredited course in Basic Life Support (BLS) (for clinical staff), or Heartsaver CPR AED (for nonclinical staff), and renewal on a regular basis, with up to a three-month grace period after the expiration date.
  • Strong communication, coaching, and problem-solving abilities
  • Ability to manage multiple priorities independently
  • Proficient in Microsoft Office Suite and related software
  • Exceptional customer service and written communication skills
  • Understanding of Medicare Conditions of Participation for Home Health and Hospice (OASIS and HOPE assessments preferred or willingness to learn)
  • Valid driver’s license and reliable transportation
  • Minimum 2 years’ experience in healthcare, with a preference for home health and hospice

  • Leadership, charge nurse, preceptor, or clinical support experience preferred
  • OASIS and HOPE assessments preferred or willingness to learn

Responsibilities

  • Provide clinical guidance for complex cases and urgent situations
  • Support interdisciplinary collaboration and smooth care transitions
  • Meet daily with affiliate discharge planners to identify potential referrals
  • Facilitate communication between hospital teams, providers, Intake, and clinical leadership
  • Review clinical information to determine eligibility for home health or hospice services
  • Build and maintain relationships with providers, and educate them on services and processes
  • Promote agency programs at community events and professional forums
  • Assist with patient care as needed and attend discharge planning meetings
  • Track referral metrics and identify improvement areas, sharing data with leadership
  • Ensure compliance with all relevant laws and regulations
  • Performs additional duties as assigned.
