Optage, a division of Presbyterian Homes and Services, is seeking a clinical professional to join its Home-office team as the Clinical Intake Coordinator to support Home Health with intake services. REPORTING STRUCTURE The Optage Clinical Intake Coordinator reports to the Optage Services Manager or designated leadership team member. The Clinical Intake Coordinator is responsible for the clinical review and coordination of the client intake process for service lines, including Home Health, Hospice, and Rehabilitation. The Clinical Intake Coordinator is responsible for maintaining positive relationships with customers and referral sources and ensuring all pertinent information is obtained and processed. This role is a clinical liaison to referral sources, providing education, evaluation, and coordination of services. Requirements

  • Licensed Practical Nurse or Registered Nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
  • Minimum of one year experience in home health, hospice, senior housing, or health care related environment.
  • Understanding of healthcare reimbursement processes, including insurance verification, prior authorizations, and payer requirements.
  • Must have strong communication, problem-solving skills, and customer experience focus
  • Must be able to work independently and multi-task with minimal supervision.
  • Must be a team player, willing to take directions and accept responsibility for meeting specified objectives.
  • Proficient with electronic medical records (EMR) systems and Microsoft Office Suite
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.
