About The Position

Join Acentra Health as a Clinical Evaluator (PASRR) in Seattle, Washington, and make a meaningful impact in your community. This hybrid role includes conducting in-person behavioral health assessments across nursing homes, hospitals, and community-based settings in Seattle, Washington. As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your work will focus on clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. These evaluations are essential for determining the appropriate next level of care and contribute to improved patient well-being and health outcomes. This full-time role offers both flexibility and purpose combining remote work from your home office with local travel. Acentra Health reimburses all travel-related expenses, including mileage and parking, and provides all required technology and equipment, including a laptop for fieldwork.

Requirements

  • Must possess an active, unrestricted license in the State of Washington in a healthcare or related field, such as Licensed Psychologist (LP), Licensed Professional Clinical Counselor (LPCC), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Professional (LMHP), Registered Nurse (RN) with Master's degree in Psychiatric Nursing or relevant field).
  • Graduation from an accredited college with a relevant degree program and a minimum of 2 - 3 years of experience in assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.
  • Proven ability to assess and analyze psychological, emotional, behavioral, and physical functioning to support accurate and thorough clinical evaluations.
  • Knowledge of Level of Care standards and best practices related to mental health, geriatrics, and long-term care needs.
  • Experience formulating clinical diagnoses for complex psychological and behavioral health conditions.
  • Effective communication skills with the ability to interact effectively with individuals and groups through verbal, written, and electronic formats.
  • Attention to detail and the ability to effectively prioritize and execute tasks in a timely manner.

Nice To Haves

  • Familiarity with Preadmission Screening and Resident Review (PASRR) requirements per State and Federal Regulation.
  • Knowledge of public mental health systems and medical programs, including public sector services and supports, with a strong understanding of Level of Care guidelines and related standards of practice.
  • Experience diagnosing psychiatric disorders using current DSM standards (state-specific).
  • Proficient in computer-based software, including Microsoft Office Suite (Word, Excel, Outlook, Teams).
  • Prior experience with the geriatric population preferred.

Responsibilities

  • Responsible for conducting PASRR Level II behavioral health evaluations, assess individuals' physical, behavioral, and psychological functioning, and develop informed level-of-care recommendations to determine the most appropriate next level of care for individuals who may require long-term care placement.
  • Assess and analyze patients' psychological, emotional, behavioral, and physical functioning to inform clinical decisions.
  • Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations.
  • Provide well-informed level of care recommendations based on clinical evaluations, ensuring adherence to established workflows, including re-evaluation of records in response to psychiatric input.
  • Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.
  • Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements.
  • Knowledge of standards of practice related to Level of Care determinations, as well as mental health, geriatrics, and other conditions commonly requiring long-term care placement.
  • Ensure compliance with established workflows, including re-evaluations of records based on psychiatric input.
  • Understand, and comply with all corporate policies, including those related to HIPAA and its Privacy and Security Rules.
