About The Position

Collaborates with the physicians and other health care team members to provide nutritional services to Advocate Health Center patients 1)Develops nutrition care plan per ADA guidelines and/or recommendation of the provider. 2)Provides individual and group dietary counseling. 3)Reviews medical record for pertinent data and discuss the nutritional care plan with nursing and medical staff as necessary. 4)Documents nutrition counseling provided and assessment of patient's understanding of information provided in the SOAP format. 5)Maintains communication with patients and significant others. 6)Orders lab based on Advocate Health Care protocol. Nutritional expert/liaison to centers 1)Develops educational materials and displays. 2)Provides in-service training to other associates. 3)Guides clinical experience of dietary student interns. 4)Participates in various committees as assigned. 5)Serves as a resource to physicians regarding nutrition. 6)Researches, plans, and develops curriculum. 7)Teaches nutrition classes. 8)Promotes National Nutrition Month in health centers. Requirements

  • Bachelors Degree in Clinical Nutrition.
  • One year clinical nutrition experience required.
  • Must be able to respond to the age appropriate psychological and physical needs of patients.
  • Understands growth and development needs of the age group with which the associate interacts.
  • Good interpersonal and communication skills.
  • Accurate with follow through.
  • ADA registration.
  • Illinois license as a licensed dietitian required.
  • CPR certification within 60 days of hire.
  • Ability to lift up to 35 pounds without assistance.
  • For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.

  • Diabetes education experience is preferred
