About The Position

Our Lady of Consolation is a proud member of Catholic Health and a recognized leader in providing exceptional quality driven care. We offer Long Term, Post-Acute and Skilled Nursing services. Our Lady of Consolation is located on the beautiful Great South Bay and is comprised of a 345-bed nursing and rehabilitative care center. Our nurses, physicians and support staff are devoted to providing patient driven health care in an environment of compassion to our patients, their families and our community. The Clinical Dietitian is responsible for assessing residents’ nutritional needs and providing individualized medical nutrition therapy to support health, functional status, and quality of life. This role collaborates closely with the interdisciplinary team to ensure compliance with CMS regulations, MDS requirements, and facility policies.

Requirements

  • Bachelor’s or Master’s degree in Dietetics, Nutrition, or related field
  • Registered Dietitian Nutritionist (RDN) credential required
  • State licensure or certification as required
  • Knowledge of CMS regulations, MDS, and survey process
  • Experience with enteral nutrition and texture-modified diets
  • Strong documentation, organizational, and communication skills

Nice To Haves

  • Long-term care or skilled nursing facility experience preferred

Responsibilities

  • Conduct comprehensive nutritional assessments for residents upon admission, quarterly, annually, and with significant change in condition
  • Develop, implement, and monitor individualized nutrition care plans in accordance with resident needs and physician orders
  • Participate in care plan meetings and interdisciplinary team (IDT) reviews
  • Complete and review MDS sections related to nutrition, weight loss, tube feeding, and therapeutic diets
  • Monitor resident weights, lab values, hydration status, and skin integrity
  • Identify residents at nutritional risk and implement appropriate interventions
  • Provide nutrition support for special populations (wounds, diabetes, renal disease, dysphagia, enteral nutrition)
  • Collaborate with nursing, speech therapy, dietary staff, and providers to ensure appropriate diet textures and consistency modifications
  • Oversee therapeutic diets, supplements, nourishments, and adaptive feeding equipment
  • Provide nutrition education to residents, families, and staff as needed
  • Ensure compliance with CMS, state regulations, survey standards, and facility policies
  • Assist with survey readiness and respond to nutrition-related citations or audits
  • Monitor food service operations as they relate to resident nutrition and regulatory compliance
