About The Position
Catholic Health Home Care Fee for Service Dietician Catholic Health Home Care Dieticians evaluate the dietary care plan of patients identified as high risk, educates patients and staff about how to optimize nutritional health and develops a plan of care for patients to reach optimal nutritional status. The Dietician works to educate patients and Home Health staff in dietary management of disease process and communicates regularly with nursing staff and the physicians as needed to discuss patient progress.
Requirements
- Bachelor's degree in nutritional sciences, community or public health nutrition (Master's preferred).
- Registered Professional Dietician in New York State and registered by the American Dietetic Association.
- Five years' experience in clinical nutrition
- Proficiency with Electronic Medical Records systems with EPIC experience a plus
