Join our Clinical Dietitian team at St. Elizabeth Healthcare! Evaluate and assess nutritional status of patients. Initiate medical nutrition therapy to achieve patient goals. Establish nutrition diagnosis and develop individualized plan of care with appropriate interventions. Re-assess / Monitor patients to evaluate the effectiveness of medical nutrition therapy and adjust patient’s plan of care. Evaluate process towards goals. Accurately assess, prescribe, and maintain nutritional interventions for enteral and parenteral supported patients. Communicate effectively with physicians, nursing, and the interdisciplinary team to coordinate patient care and timely discharge. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served including: newborn, infant, pediatric, adolescent, adult, and geriatric patients. Serve as a resource lecturer for healthcare providers, students, and the community. Precept dietetic interns for supervised practice experience. Quality: Compliance with all local, state, federal, and medical center regulatory, safety, sanitation, and security standards. Participates in performance improvement (PI) studies, assist with data collections and supports action plans. Performs other duties as assigned.
