Requirements
- Active RN license (Indiana)
- Long-term care experience
- Strong attention to detail and organization
Responsibilities
- Complete MDS assessments and care plans
- Coordinate Medicare (PPS) schedules and timelines
- Ensure documentation meets regulatory standards
- Collaborate with interdisciplinary teams
- Monitor and respond to changes in resident condition
