About The Position

The Fortune Society is seeking a participant-centered, strategic thinker to join our agency as we partner with the Bronx Criminal Court (BCC) to launch an innovative new referral-based program for women who are facing charges for non-federal offenses. Working alongside both judicial and non-judicial members of the BCC, and under the supervision of Fortune’s Behavioral Health clinical team, the Clinical Case Manager will be responsible for screening women at the point of arraignment to identify those in need of mental health services and/or treatment as well as those who may be victims of human trafficking, sexual exploitation, and/or domestic or sexual violence, and connecting them to services as appropriate. The Case Manager will serve as the initial point of contact, responsible for candidate screenings and for making appropriate, real-time referrals. The Clinical Case Manager must be able to demonstrate an ability to identify primary treatment/intervention needs in women, especially mental health disorders, substance abuse and other areas of concerns and rapidly connect them to appropriate internal/external referral sources. The Case Manager will work directly with the court’s resource coordinator / project director and report to the Presiding Judge of the Bronx Misdemeanor Mental Health Court. Candidates must have experience in the mental health and substance abuse fields, as well as strong organizational skills.

Requirements

  • Master's Degree in criminal justice, social work or related field with substantial experience working with populations experiencing mental illness, commercial sexual exploitation, labor/sex trafficking, gender-based violence, and/or sexual assault, and knowledge of trauma-informed care is required. An equivalent combination of education and experience will also be considered
  • Demonstrated ability to work cooperatively with treatment and criminal justice systems

Nice To Haves

  • Preference will be given to candidates who are fluent in a second language.
  • Relevant personal experience is a plus.

Responsibilities

  • Conduct screenings of women arraigned in the Bronx Criminal Court (BCC) to identify potential participants so their case(s) may be referred to the appropriate problem-solving court for further screening and assessment
  • Flag the cases of those women who are appropriate candidates for a mental health court, human trafficking intervention court, drug court, or other problem-solving court
  • Serve as a resource for judges and court personnel in the BCC on questions related to mental health services and treatment for women
  • Serve as a liaison between the BCC and Bronx problem-solving courts
  • Identify and nurture referral relationships with both internal and external services providers in an effort to make the most accurate and efficient referral connections for participants
  • Develop a referral tracking tools and maintain accurate notes, including status of the referral and follow up activities
  • Work with Fortune’s data analytics team to produce and quantify key data such as # of internal/external referrals, # and type of service connections made, and length of engagement
  • Assist in Maintaining and tracking data entry for GPRAs at baseline, 6-month follow-ups and discharge
  • May be required to be available to court staff (remotely) for evening and/or weekend assistance on a rotational basis with other team member(s)
