About The Position

The Clinical Case Manager is responsible for providing community-based outreach to link or re-link clients to services needed and provide support through recovery goals that prevent risk of hospitalization or rehospitalization. CCM's provide timely support and links to necessary services, while assisting clients in improving their quality of life by supporting enhanced relationships, empowerment, integration into the community, and assisting clients with realizing their potential.

Requirements

  • Bachelor’s Degree in Social Work, Psychology or related field, or AA Degree and four years’ experience working with individuals with mental illness
  • A minimum of one year of experience working in the mental health field post graduation, or four years experience with an AA Degree
  • Driver's License in good standing
  • Demonstrates ability to work on a team and ability to work with other disciplines in a team environment.
  • Demonstrates ability to telework and work independently with remote support at times.
  • Demonstrates ability to perform basic computer functions and use of electronic records as required by job function.
  • Completion of an individual recovery plan in collaboration with consumers.
  • Demonstrates the ability to complete other documentation, notes, Intakes, discharge summaries, etc. as required.

Responsibilities

  • Individual needs of clients are addressed including assessment and evaluation, referral, follow-up services, individual, group, and family therapy.
  • Demonstrates ability to assess clients' needs, and provide interventions that are timely and appropriate.
  • Demonstrates ability to assess risk category, mental status, level of compliance and motivation.
  • Case managers shall provide psycho education that may include relevant education about the clients' disability or disorder, family interactions, community integration, relationships, or importance of living skills Assisting clients with accessing transportation, securing appropriate housing, and exploring employment or other meaningful activities.
  • Coordination, integration, and monitoring of services delivered from multiple providers until client is discharged from ICMS are essential to the advocacy process.
  • Responsible for self-monitoring of daily productivity to consistently meet weekly target for units of service.
  • Demonstrates ability to work on a team and ability to work with other disciplines in a team environment.
