Who we are looking for: Detail-oriented Certified Nursing Assistants (CNA), Medical Assistants (MA), Licensed Practical Nurses (LPN), and Registered Nurses (RN) to support Care Managers and UWH providers by managing dashboards, tracking referrals, and keeping communication moving across post-acute settings. This role includes patient and family outreach and coordination of services to support smooth transitions of care. This position does not involve independent clinical assessments.
