About The Position

The Long Term Care Clinical Coordinator is responsible for coordinating the MDS process as well as day to day clinical management of resident care. This position supervises team leaders, medication aides and CNA’s to ensure resident care is delivered. General Hours of Work Monday through Friday. Must be flexible to meet the needs of the Care Center.

Requirements

  • RN or LPN degree required. If LPN, RN license needs to be obtained within 3 years from the date of hire.
  • Two or more years experience in long term care and/or supervisor experience.
  • Has experience with Microsoft Office and basic PC experience.
  • Must have current Iowa license to practice as a Licensed Practical Nurse or Registered Nurse.
  • Must obtain CPR certification within 3 months of hire date.

Responsibilities

  • Assumes overall nursing direction of the nursing facility in the absence of the Long Term Care Manager.
  • Ensures accurate assessment and documentation of new admissions.
  • Completes accurate assessments on all MDS’s and ensures computer coding is accurate for proper reimbursement.
  • Stays up to date on MDS and changes related to current rules.
  • Maintains accurate records to ensure the timely completion of CAA’s, Care Plans and Care Conferences.
  • Ensures and properly performs ongoing resident assessment for alterations in resident health status and documents accurately and properly.
  • Ensures each resident has opportunity to be part of the restorative therapy program.
  • Assesses each resident using the MDS and the resident assessment protocols. Any resident with ADL rehab potential, urinary incontinence, limited range of motion, and risk for impaired tissues, integrity, and/or difficulty with eating will be evaluated for the restorative program.
  • Ensures timely dissemination of care plan interventions and physician orders to team.
  • Assesses and provides necessary and appropriate education to resident, families, co-workers, and students.
  • Assesses and initiates consultation as appropriate throughout progression of care.
  • Monitors residents for change in level of care/condition.
  • Maintains competency in charge nurse role, and is able to fill in when needed.
  • Is a leader that demonstrates a hands on approach to cares with the team; rounding on residents and modeling a team approach in all resident cares.
  • Assists in the managerial responsibilities (interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems).
  • Performs other duties and responsibilities as assigned. Includes working on the floor to cover open shifts and when short staffed.
