Responsible for providing care to patients under the direction of the quality leadership. The Coordinator Clinical Care's primary responsibility is to administer care coordination activities for the patients of medical groups. This includes monitoring care coordination processes and support primary clinical teams through entering standing orders to facilitate screenings and lab tests as well as engaging patients with preventative and chronic care gaps. It will also include identifying the high priority patient population and working more directly to ensure care coordination for this patient population. The Coordinator Clinical Care behaves in a professional manner, and consistently demonstrates and promotes the values of Providence. The Coordinator Clinical Care is the liaison between providers, primary care clinics, specialist offices, patients, ancillary departments (e.g., Laboratory), and freestanding care delivery centers or services for population health programs and related processes.
