About The Position

The Residential team provides mental health residential treatment for adults with acute mental health needs who have not responded to outpatient treatment, have needs that cannot be met in a less-restrictive setting, or would benefit from further treatment following inpatient psychiatric hospitalization. The team provides medication services and individual and group services with a focus on skill development including communication, problem solving, and conflict resolution; life and social skills to restore functioning; and self-regulation, anger management, and other mood management skills.

Requirements

  • High school diploma or GED. (Associate degree applicants greatly encouraged to apply)
  • Two years of case management experience working with mentally ill adults in relevant settings such alcohol, drug, developmental disability, physical rehabilitation, etc.
  • Valid driver's license with a 5-year clean driving record

Nice To Haves

  • Preference will be given to candidates who have completed Peer Support Training and have experience in a Clinical Associate/Case Management/Peer Support Role.
  • Additional preference on individuals who have completed SOAR Adult Curriculum.

Responsibilities

  • Provide assertive outreach and engage individuals in the community "where they are at" and work to address barriers to housing and improve social welfare and healthcare access/ outcomes.
  • Provide case management support to clients in connecting to community resources as well as active skill building, focusing on skills to address functional impairments and to enhance the quality of life
  • Assist clients in meeting project goals; to include areas of management of daily routine/skills building, applications for housing, employment/social welfare, healthcare, and referring to needed treatment or community resources.
