Requirements
- Graduate of School of Pharmacy.
- Advanced degree (Doctor of Pharmacy preferred) with residency training or equivalent work experience required.
- Experience in ambulatory setting with disease state management preferred.
- Current applicable state Registered Pharmacist License (RPh) required.
Responsibilities
- Manage patients in the ambulatory care setting through a collaborative, team-based model involving providers, clinic staff, resource specialists, and clinical resource coordinators.
- Provide disease state management services to support chronic condition care and therapeutic outcomes.
- Deliver patient education to improve understanding, adherence, and self-management of prescribed therapies.
- Optimize drug therapy regimens in partnership with providers, ensuring safe, effective, and cost-conscious medication use.
- Contribute to the design and implementation of clinical initiatives and care models in collaboration with clinic teams.
