(Required) 1 - 2 years of experience working with a frail or elderly population or, in the absence of such experience, receive appropriate training from PACE on working with a frail or elderly population before providing direct patient care to PACE participants independently. Relevant experience must be within the last 3 years. (Required) 3 - 4 years of LPN clinical experience. (Preferred) Other Current LPN license in good standing to practice in Ohio. Upon Hire Required Basic Life Support (BLS) Required Job Overview: The PACE LPN provides general nursing care to PACE participants, executes nursing interventions under the supervision of the PACE primary care providers and RN nursing leadership staff, and collects data on participant medical conditions in the PACE clinic and by telephone. The PACE LPN provides nursing education to participants, caregivers, and PACE staff. The PACE LPN is responsible for scheduling participant appointments for PACE Clinic referrals made to medical specialists and ancillary care providers in the community.
