About The Position

The Licensed Practical Nurse serves as a critical member of a care team within at New Orleans East Louisiana Community Health Center supporting team-based care alongside providers, Medical Assistants, Social Workers and Care Coordinators. The LPN delivers nursing care, supports chronic care management, Provider extension and provides patient education to improve outcomes and access for underserved populations, while ensuring compliance with HRSA standards.

Requirements

  • Current Licensed Practical Nurse license in Louisiana.
  • Graduate of an accredited practical nursing program.
  • Current BLS certification.
  • Basic experience with electronic health records.
  • Licensed Practical Nurse Certificate.
  • 2-4 years of full-time clinical experience as an LPN, particularly in primary care for adult, pediatric, OB-GYN and geriatric patients.
  • Knowledge of primary care nursing, care management principles, chronic disease management, and preventive care.
  • Skill in patient assessment, patient education, and coordination of care within an interdisciplinary team.
  • Ability to use electronic health records and standard office software.
  • Strong written and verbal communication skills and attention to detail.
  • Demonstrated commitment to health equity, cultural humility, and trauma informed care

Responsibilities

  • Provide nursing care including assessments, education medication administration, immunizations, and treatments within scope of practice.
  • Act as a provider extender by carrying out standing orders, reinforcing care plans, and supporting efficient patient flow.
  • Support management of common chronic conditions and preventive services such as immunizations and screenings.
  • Assist providers during exams and procedures.
  • Identify care gaps and support follow-up activities.
  • Coordinate care with Providers, Medical Assistants, Care Coordinators, and Social Workers.
  • Participate in team huddles and quality improvement efforts.
  • Maintain infection control, patient confidentiality, and compliance with HRSA and organizational standard .
  • Monitor patients after medications or immunizations and report concerns to providers.
  • Document patient encounters accurately in Athena EHR and follow clinic protocols and regulatory requirements.
  • Participate in quality improvement efforts including immunization tracking and preventive care measures.
  • Participate in occasional community outreach or health education events.
