About The Position

Assess client’s level of need and condition of surroundings relevant to determination of eligibility for home delivered meals program. Identify available resources to make necessary referral(s) to support clients as appropriate. Works closely with other coordinators to coordinate assessments, documentation, and files as needed. Works in coordination with Driver III’s and Community Engagement Team to route new clients and solve any arising issues with clients, drivers, and routes. This position is responsible to the Client Services Coordinator Supervisor.

Requirements

  • Bachelor’s Degree preferred.
  • Bilingual (Spanish/English) preferred.
  • Communication skills in verbal and written communication.
  • Good time management and map reading skills.
  • Excellent office administration skills with an advanced knowledge of Microsoft Office programs.
  • Must have reliable, personal vehicle and a valid Driver’s License, good driving record, and applicable insurance coverage required by the company policy

  • Have mobility to work in a standard office setting and to use standard office equipment, including a computer; vision to read printed materials and a computer screen; and hearing and speech to communicate in person and over the telephone.
  • Works in a highly stressful environment dealing with a wide variety of challenges, deadlines, and diverse employee population.
  • Position requires travel to client’s homes and will be subjected to changing temperatures and possible hazardous living condition such as poor housing and vermin.
  • Be able to move freely in and out of vehicle and climb stairs as needed to deliver meals and/or complete client assessments.
  • Able to occasionally lift to twenty-five (25) pounds when delivering meals or moving office supplies or equipment.

Responsibilities

  • Assess and reassess clients for program eligibility as dictated by the government standards and Meals on Wheels policy.
  • Use your own vehicle to travel to the applicants’ and clients’ homes to complete assessments.
  • Maintain all aspect of the client’s files; completing assessment and related paperwork, follow-up on required documents or information needed and making updates to the clients’ files in a timely manner.
  • Responsible for scheduling appointments by prioritizing clients based on need and date of referral, completing an adequate number of assessments within the designated time, and keeping a daily log of home visits and mileage.
  • Responsible for referring clients to other Meals on Wheels San Antonio programs when appropriate and giving clients the opportunity to make a financial contribution towards the cost of their meals as required by OAA.
  • Communicate with clients and family/support systems on any issues that may arise with the clients’ condition, program status or referral(s) made within agency confidentiality policies.
  • Coordinate with coworkers to ensure that all assessments are completed in a timely manner.
  • Share duties with other coworkers such as client filing and data entry, as needed.
  • Appropriately screen incoming referral calls for program eligibility and make referrals on the client’s behalf.
  • Achieve and maintain a minimum of 3% client-based donations.
  • Be available to deliver meals if needed
  • Other duties as assigned by the Client Service Coordinator Supervisor or Client Services Manager.
