About The Position

Under the supervision of a Coordinator or Supervisor, serves as a professional performing a wide range of direct and indirect mental health services within Older Adult Services to consumers with chronic mental illness and possibly developmental disability and/or substance abuse issues. This position will be a half-time position between Older Adult Services Day Program and Case Management. Provides case management; wellness coaching, outreach to consumers with multiple needs; and mental health services, and crisis interventions. The position recommends referrals to various programs, program components and other services in the community. Day Program duties include a variety of supportive services and direct interventions with consumers in accordance with treatment plans developed by CMHA-CEI personnel. Responsible for detailed case documentation. Responsible for carrying out all activities of the program/sub-unit in which such a manner that fulfills CMHA-CMEI’s mission, policies and procedures.

Requirements

  • Bachelor’s degree in Social Work, Psychology or other related field required.
  • A minimum of one year of professional experience specific to the area of assignment is required.
  • On a case-by-case basis, may assess and consider an applicant with two years of paraprofessional work experience with the assigned population.
  • Must be able to operate a computer and have the ability to gain proficiency in CMH’s computer systems.
  • Possession of a valid Michigan driver’s license and access to reliable transportation for job related use is required.
  • Employment offers for this position will be contingent upon the results of a Michigan State Police clearance check, and verification of an acceptable driving record.
  • An individual may be excluded from consideration based upon their driving record.
