About The Position

The Community Health Worker (CHW) Coordinator is an operational support role designed for an experienced Community Health Worker who brings strong knowledge of ECM workflows, documentation standards, and member engagement — without direct fieldwork or event facilitation. This role focuses on care coordination support, authorizations, outreach workflows, documentation quality, and system accuracy, enabling case-carrying CHWs to focus on high-touch member relationships. The CHW Coordinator member health assessments and referrals are not conducted in-person work or events and works fully remotely.

Requirements

  • CHW certification or equivalent lived/field experience preferred.
  • 2–4+ years of Community Health Worker or ECM-related experience.
  • Strong working knowledge of ECM models, care coordination workflows, and health plan requirements.
  • Experience with Care Director, Pathways, or similar care management systems strongly preferred.
  • High attention to detail and strong documentation and compliance skills.
  • Ability to work independently, manage competing priorities, and collaborate across teams.

Responsibilities

  • Operational & Authorization Support Submit, track, and maintain HPSJ and other health plan authorizations, ensuring timely updates and accurate status tracking in Pathways. Monitor authorization timelines, follow up on pending items, and escalate delays as needed.
  • Member Outreach & Enrollment Support Pull and outreach IEHP members in Care Director in alignment with program priorities. Conduct outreach, welcome calls, consent collection, and initial assessments for newly enrolled members. Coordinate with assigned case-carrying CHWs and Astrana LCM to facilitate warm handoffs.
  • Documentation & Systems Management Ensure accurate, complete, and timely documentation in Care Director, member portals, and Pathways. Audit member records to confirm compliance with ECM and health plan documentation standards. Identify gaps, trends, or training needs related to documentation quality.
  • Care Team Collaboration Attend IDT meetings for assigned members and support follow-up actions. Serve as an operational liaison between CHWs, care management partners, and operations leadership.
  • Quality & Process Improvement Support continuous improvement by identifying workflow inefficiencies and recommending process enhancements. Assist with onboarding or up-skilling operational team members into CHW-adjacent responsibilities, as appropriate.
