The Community Health Worker/Patient Navigator (CHW) will provide patient navigation and care coordination services for individuals undergoing or eligible for Hepatitis treatment. Support patients in engagement and retention in care, including appointment scheduling, follow-up, and coordination with clinical and support services. Assist patients with treatment adherence through education, reminders, and supportive coaching using a trauma-informed and patient-centered approach. Coordinate laboratory testing, referrals, and follow-up appointments relate to Hepatitis care. Assist with prior authorizations and care coordination activities related to Hepatitis treatment, in collaboration with clinical staff and pharmacy partners. Maintain regular contact with patients through outreach efforts to re-engage individuals who are out of care or overdue for services. The CHW will support the care team working collaboratively with interdisciplinary care team members, including providers, nurses, case managers, pharmacists, and external partners. Support program goals related to viral suppression, engagement in care, and improved health outcomes. The CHW will document all services as required and will assist in meeting all program requirements.
