The Chronic Disease Registered Nurse provides direct and indirect nursing care in a family practice setting. The RN will provide assessments, disease management, education, training and other clinically based activities to coordinate care among providers, patients and the community. The RN will participate in all aspects of care to patients including triage, health maintenance teaching, and administration of vaccines/medication. The RN will also collaborate and communicate with other nursing professionals, medical assistants, medical providers and community health workers, to ensure high quality patient care.
