Job Description: Chronic Care Management Registered Nurse Location: Bohemia, NY Hours: Monday- Friday Organization: New York Cancer and Blood Specialists In This Role, You Will: Establish, implement, and revise patient and disease-centered care plans inventory the patient’s physical, mental, cognitive, psychosocial, functional, and environmental determinants of health Create comprehensive care plans based on the patient's health. lead LPNs and MAs in the implementation of a written plan of care. Utilize standing protocols to initiate therapeutic measures and call for crisis intervention when appropriate.

Requirements

  • Graduate from an accredited School of Nursing and an unencumbered New York license.
  • Nursing Experience which one has acquired clinical competence
  • Excellent assessment, communication and organizational skills.
  • Good computer knowledge.
  • The ability to plan, organize, develop, and interpret health data, goals, and objectives that are necessary for providing quality coordination of care.

Nice To Haves

  • Bi-Lingual, English/Spanish a plus.
  • HemOnc experience preferred.
