About The Position

The Children’s Health Licensed Clinical Social Worker performs clinical surveillance as well as primary and secondary level assessments and provision of counseling services to assess and respond to patients’ developmental, behavioral or mental health needs. Essential Functions include: Receives referrals from clinicians in the Children’s Health clinics to assess patients’ developmental, behavioral and mental health needs. Work as part of an integrated medical home to include pediatric medical providers, dieticians, social workers and referral coordinators. Develops and implements treatment plans and prioritizes goals, works with parents and children ages birth to 21. Serves as information and referral specialist. Refers patients to resources to facilitate access to services needed. Services may include: basic necessities, mental health, children’s health and social services programs, domestic violence resources, contact with schools, and other services as appropriate. Maintains referral lists. Assists with case management for patients who need primary care services. Provides ongoing counseling and psycho-educational support to patients and parents. Provides crisis services by managing mental health emergencies when they arise. Manages ADHD screening program, works closely with referral coordinator and school personnel to complete process. Attend meetings for foster care program, medical provider meetings and staff meetings. Collaborate with outside agencies to best serve patient population. Documents activities. Completes assessments, follow-up notes, and phone notes within specified deadlines. Completes inquiry reports to obtain monthly statistics and submit them to assigned supervisor. Verifies demographic information and patient information for clients. Monitors and responds to referrals and flags. May manage billing information. Requirements

  • Clinical Social Work License
  • Please include ALL prior work experience on your application and resume.
  • Thorough knowledge of social work principles, techniques, and practices, and their application to complex casework
  • Considerable knowledge of a wide range of behavior and psychosocial problems and their diagnosis and treatment
  • Considerable knowledge of family and group dynamics and a range of intervention techniques, as well as principles of casework and training
  • Ability to formulate appropriate goals and interventions
  • Considerable knowledge of treatment planning
  • Skill in establishing rapport with a client, establishing an effective interpersonal relationship
