About The Position

Works with people with qualifying Physical/Mental Health/Substance Abuse/Developmental Disability diagnoses and their natural supports to link/monitor/refer to services that meet their individual needs and personal goals. Also provides ongoing advocacy. Provides ongoing advocacy through person centered approach. Meets documentation requirements of regulating organizations by completing documentation.

Requirements

  • Associates' Degree in a Human Services related field.
  • One to Two (1-2) years of relevant experience in Human services (including but not limited to Mental Health, Social Work, Substance Use, Education, etc.)., with a minimum of 6-12 months experience developing Care Plans/SMART Goals and functional knowledge of community resources.
  • For applicants with an Associate’s degree, a waiver is required to be completed and accepted by The New York State Department of Health before hire.
  • Valid New York State driver’s license and dependable transportation that meets the Agency standards
  • Ability to work flexible hours and travel throughout Counties Served: Allegany, Cattaraugus, and Chautauqua (Caseloads may cover two adjacent counties)
  • Ability to work effectively and independently including address multiple tasks efficiently
  • Strong organizational and analytical skills
  • Ability to think creatively and be results oriented
  • Ability to effectively communicate verbally and in writing including developing rapport with the people we serve
  • Willingness and ability to participate in an On-all rotation

Nice To Haves

  • Bachelor’s Degree of Arts or Science
  • Two (2) years of relevant experience in Human services (including but not limited to Mental Health, Social Work, Substance Use, Education, etc.), with a minimum of 6-12 months experience developing Care Plans/SMART Goals and functional knowledge of community resources.
  • Applicants with a Bachelor’s Degree with less than 2 years’ experience may be considered, but a waiver is required to be completed and accepted by The New York State Department of Health before hire
