About The Position

TrueCare is a Mississippi non-profit, provider-sponsored health plan formed by a coalition of Mississippi hospitals and health systems throughout the state and supported by CareSource’s national leadership in quality and operational excellence. TrueCare offers locally based provider services through provider engagement representatives and customer care. Our sole mission is to improve the health of Mississippians by leveraging local physician experience to inform decision-making, aligning incentives, using data more effectively, and reducing friction between the delivery and financing of health care. By doing so, TrueCare will change the way health care is delivered in Mississippi. Job Summary: The Child Welfare and Community Justice Liaison is responsible for engaging with members prior to their release from incarceration and during the transition period as these members re-enter their community. The Liaison will assist in delivering a coordinated re-entry experience for our members through collaboration with the Department of Correction and internal and external stakeholders. The Liaison will help ensure end-to-end care coordination by engaging with incarcerated members prior to release, obtaining information on the physical, behavioral, and social needs of the member to assist them with goals and resources for success.

Requirements

  • Bachelor's degree in social work, health care, business or related field required
  • Equivalent years of relevant work experience may be accepted in lieu of required education
  • Two (2) years of care coordination or related experience required
  • Proficiency with Microsoft Office Suite to include Word, Excel and PowerPoint
  • Excellent written and verbal communication skills
  • Excellent active listening and critical thinking skills
  • Strong problem-solving skills with attention to detail
  • Strong interpersonal skills and high level of professionalism
  • Ability to work independently and within a team environment
  • Ability to develop, prioritize and accomplish goals
  • Knowledge of regulatory requirements
  • Knowledge of health plan policies and community-based organization that can impact outcomes
  • Strong relationship building and management skills
  • Must have valid driver's license, vehicle and verifiable insurance.
  • Employment in this position is conditional pending successful clearance of a driver’s license record check.
  • To help protect our employees, members, and the communities we serve from acquiring communicable diseases, Influenza vaccination is a requirement of this position. CareSource requires annual proof of Influenza vaccination for designated positions during Influenza season (October 1 – March 31) as a condition of continued employment. Employees hired during Influenza season will have thirty (30) days from their hire date to complete the required vaccination and have record of immunization verified.

Responsibilities

  • Provide in-reach education and engagement services to incarcerated youth and staff in the Mississippi Department of Correction.
  • Engage with the member in a variety of settings to establish an effective, professional relationship. Settings for engagement may include but are not limited to a correctional facility, hospital, provider office, community agency, telephonic or electronic communication
  • Responsible for interfacing with the existing care coordination continuum, including the member, Mississippi Department of Corrections, TrueCare care management and Child Welfare staff, community health partners and community agencies to foster the transition of the member back into their community.
  • Ensure the creation of a care plan and help coordinate the connection of the member to community providers and resources
  • Maintain appropriate documentation
  • Responsible for gathering member data
  • Participate in relevant State Agency and advocacy meetings: Including but not limited to Dept of Corrections & Child Protection Services
  • Contribute to operational and strategic feedback to programmatic development
  • Responsible to serve as an interdepartmental and inter-market liaison to gather/share information about issues and barriers related to the successful execution of our program
  • Act as a liaison to government agencies and social service/community resources to communicate ideas, address concerns and relay solutions
  • Educate members and family members / caretakers through implementation of tools, processes, and competencies
  • Perform any other job related duties as requested.
