About The Position

Bend Transitional Care is a leading skilled nursing and rehabilitation center committed to providing exceptional care in a safe, comfortable, and welcoming environment. We’re seeking a dedicated and experienced Charge Nurse (RN or LPN) to join our nursing team! In this key leadership role, you’ll oversee daily nursing operations, ensure clinical excellence, and support our residents in achieving their highest level of health and independence. You’ll guide nursing staff, manage patient care plans, and collaborate closely with physicians, families, and interdisciplinary teams to promote quality outcomes. Requirements

  • Current Oregon RN or LPN license.
  • Graduate of an accredited school of nursing.
  • Valid CPR certification.
  • At least one (1) year of nursing experience in a skilled nursing or long-term care setting preferred.
  • Strong leadership, communication, and critical-thinking skills.
  • Ability to manage multiple priorities and maintain composure in fast-paced environments.
  • Compassionate, patient-focused approach with a commitment to teamwork and integrity.
  • Graduate of accredited LPN School of Nursing.
  • Current valid LPN license in the state employed.
  • Valid CPR certification.
  • Six months experience in a long-term care environment preferred.

Responsibilities

  • Supervise and support nursing staff to ensure high-quality care and adherence to care plans.
  • Administer medications and treatments in accordance with physician orders and regulatory standards.
  • Assess residents’ conditions, monitor changes, and communicate updates promptly to the care team and families.
  • Coordinate with physicians, therapists, and other healthcare providers to ensure comprehensive care.
  • Provide guidance and mentorship to CNAs and nurses, fostering teamwork and accountability.
  • Maintain accurate and timely documentation in residents’ medical records.
  • Respond promptly to emergencies, implement appropriate interventions, and follow up on outcomes.
  • Participate in shift reports, rounds, and care plan meetings.
  • Uphold infection control, safety, and compliance standards at all times.
  • Perform other related duties as assigned.
