About The Position

The Change Management Practitioner will lead and support organizational change initiatives across our healthcare system. This role is critical in ensuring successful adoption of strategic programs, including clinical transformation, digital health implementations, and operational excellence efforts. The Change Management Practitioner will have a strong background in healthcare operations, stakeholder engagement, and change enablement strategies.

Requirements

  • Bachelor's degree in Healthcare Administration, Organizational Development, Business, or related field.
  • Prosci or equivalent change management certification required.
  • Minimum five (5) years experience in change management, with at least 3 years in a senior or lead role within a hospital or health system.

Responsibilities

  • Develop and execute comprehensive change management strategies for enterprise-wide healthcare initiatives.
  • Collaborate with clinical, administrative, and IT teams to ensure alignment and smooth transitions.
  • Conduct impact assessments, stakeholder analyses, and readiness evaluations to inform change plans.
  • Design and deliver communication plans, training programs, and reinforcement strategies to drive adoption.
  • Facilitate workshops, focus groups, and feedback sessions to foster engagement and continuous improvement.
  • Monitor and report on change effectiveness, adoption metrics, and feedback loops.
  • Support the integration of change management into project and program lifecycles.
  • Serve as a trusted advisor to leadership, guiding teams through change with empathy and clarity.
