About The Position

VNA Community Services has provided compassionate, community-centered care throughout Montgomery, Bucks, and Philadelphia Counties for over a century. Our mission is to empower individuals and families to live with dignity, independence, and confidence - delivering high-quality primary care and supportive services across all ages. As we continue to grow, we are seeking a dedicated, experienced Certified Nurse Practitioner to join our Family Health Center team and help expand access to comprehensive, patient-centered care.

Requirements

  • Master's or Doctoral degree in Nursing (or equivalent) from an accredited Nurse Practitioner program.
  • Active Nurse Practitioner license in the state of Pennsylvania (or relevant state licensure) and national certification (e.g., family practice or adult-gerontology).
  • Strong clinical judgment, diagnostic ability, and comfort managing a broad spectrum of health conditions and patient populations.
  • Excellent communication skills, with a patient-centered and compassionate approach.
  • Capacity to work independently and collaboratively as part of a multidisciplinary care team.
  • Familiarity with electronic health records (EHR) and documentation best practices.

Responsibilities

  • Provide comprehensive primary care services to patients of all ages (infants through seniors), including wellness exams, acute care, and chronic disease management.
  • Conduct patient assessments, diagnose illnesses, order/inter­pret diagnostic tests, and develop and implement treatment plans.
  • Prescribe medications and therapies, including managing ongoing medication regimens.
  • Provide patient education, counseling, preventive health guidance, and support for health maintenance.
  • Collaborate closely with other healthcare professionals - including RNs, support staff, and administrative team - to ensure coordinated, high-quality care.
  • Maintain accurate patient records and documentation, including use of electronic health record (EHR) systems.
  • Participate in ongoing professional development, training, and quality-improvement initiatives to support our mission of compassionate, patient-centered care.
