Hospice Certified Registered Nurse Practitioner - PRN

We are seeking a dedicated and compassionate PRN Certified Registered Nurse Practitioner (CRNP) to join our hospice care team. In this role, you will be a vital member of the Interdisciplinary Care Team (IDT), working collaboratively with the Medical Director, physicians, and other healthcare professionals to provide exceptional, patient-centered hospice care. This position is ideal for a highly motivated and organized practitioner who thrives in a collaborative environment and is passionate about improving the quality of life for patients and their families.
Requirements
- Graduate of an accredited Nurse Practitioner program
- Current national certification as a CRNP
- Strong knowledge of hospice care principles and practices
- Familiarity with Medicare Face-to-Face Encounter criteria
- Excellent clinical assessment, diagnostic, and treatment skills
- Ability to work effectively within an interdisciplinary team
- Outstanding communication and organizational skills
- A genuine passion for compassionate, patient-focused care
Responsibilities
- Provide comprehensive medical care and assessments in both facility and home settings
- Perform physical examinations and preventive care under physician guidance
- Order, interpret, and evaluate diagnostic tests to assess patient conditions
- Develop and document treatment plans, findings, and prognoses
- Prescribe medications and recommend treatments as appropriate
- Participate in ongoing care coordination and case discussions
- Ensure compliance with Medicare Face-to-Face Encounter requirements
