About The Position

Family First Health is a federally qualified community health center that understands that not every individual or community shares the same needs. That’s why we provide primary medical, dental, and behavioral health care at seven convenient locations in diverse geographic regions across Lebanon, York, Adams, and Lancaster Counties to serve our patients. Family First Health also offers social service programs for those living with HIV/AIDS and first-time mothers, as well as substance use service programs. Our goals include promoting and supporting healthy lifestyles, collaborating with others to improve access and break down the barriers of affordability, language and culture; and advocating for our patients and the medically underserved. The role of the Certified Registered Nurse Practitioner is to provide the full scope of primary medical care to patients including diagnosis and treatment of a broad range of conditions following standard and accepted protocols and procedures. The Physician will also document patient visits and monitor and facilitate other activities related to patient care. The Nurse Practitioner will work alongside their support staff team to treat newborn to geriatric patients and multiple generations of the same families in an outpatient setting. In addition, the Nurse Practitioner will need to be committed to safe, ethical, and excellent clinical practice; and have the desire to make a difference in the community that they serve.

Requirements

  • Graduation from an accredited Certified Registered Nurse Practitioner program
  • Active Pennsylvania licensure for a Certified Registered Nurse Practitioner
  • Current CPR and Narcan certification
  • Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and deformities, including symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures
  • Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders
  • Knowledge of principles and processes for providing patient services.
  • Knowledge of principles, methods, and procedures for diagnosis and treatment
  • Understanding the implications of new information for both current and future problem-solving and decision-making
  • Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times
  • Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems
  • Being aware of others' reactions and understanding why they react as they do

Responsibilities

  • Provides comprehensive medical care for the patients of Family First Health in consultation with staff physicians and within the prescribed scope of practice as detailed in the PA Nurse Practitioner Acts
  • Provides primary medical care including diagnosis and treatment of a broad range of conditions following standard and accepted protocols and procedures, using standard and accepted instruments and equipment to ensure the delivery of standard of care under the general supervision of a staff physician
  • Complies with oversight by collaborating physician and works within the scope of the midlevel licensure
  • Documents patient visits and reviews incoming documentation in a timely manner
  • Provides input to the formulation of patient care policies and procedures for the department and organization.
  • Facilitates and monitors activities related to patient care and sets standards for accurate reporting and recording of patients’ symptoms, reactions and progress
  • Organizes patient care and ensures continuity of care based upon knowledge of patient needs
  • Evaluates care through assessment, planning, intervention, re-evaluation, written documentation, observation and comparison to established standards
  • Guides nursing staff as necessary and participates in training; delegates appropriate assignments to nursing staff
  • Participates in peer review process
  • Provides patient education on treatments, medications, diet and other disease and illness prevention or management strategies
  • Attend and actively participate in key organizational meetings
  • Performs all other duties assigned
