This position is for a part time or full time Certified Peer Recovery Support Specialist (CPRS) and such offering shall be at the discretion of Cedar Recovery based upon the needs of the company and request of the potential candidate. Responsibilities include: Provide recovery education and support to Cedar Recovery’s patients at all phases of treatment Engage with patients at hospitals through motivational interviewing tactics and coordinate care for those patients to the appropriate treatment setting(s) Facilitate recovery planning activities with patients Identify and coordinate community resources for patients (housing, food, work, education, relationships, etc.) Coordinate care with providers Act as a community liaison for Cedar to develop referral relationships and promote the reputation of Cedar in the areas Cedar serves Link individuals to appropriate community-based groups and resources Remain connected by phone or email to Cedar’s patients for purposes of ensuring their on-going success and engagement in their respective program Lead individual or group peer recovery sessions as directed by the peer’s supervisor
